Responsibilities Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support. Monitor patient health by observing vital signs, patient behavior, and overall condition; report changes promptly to healthcare professionals Support medication administration following care plans and HIPAA regulations to ensure privacy and safety. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Use proper manual handling techniques and patient transfer methods, including Hoyer lifts and other equipment, to ensure safety during transfers. Maintain a clean and safe environment through regular cleaning duties and adherence to infection control protocols Document care activities accurately in compliance with facility policies and regulatory standards Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as nursing homes or assisted living facilities Strong knowledge of dementia care, Alzheimer's care, memory care, and hospice care practices Familiarity with patient observation techniques, patient monitoring, and residents' rights protections Ability to assist with complex tasks like medication administration, vital signs measurement, CPR certification, first aid skills, and use of Hoyer lifts Excellent communication skills for engaging effectively with clients, families, and healthcare teams Experience working within home & community care settings or long-term care environments is highly desirable Physical stamina for heavy lifting, assisting with transfers, manual handling, and providing personal transportation assistance when needed This role is ideal for compassionate individuals eager to support others through personalized caregiving.
