Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods like Hoyer lifts when necessary. Monitor patients' vital signs, observe health status changes, and accurately document patient observations to ensure timely interventions. Prepare nutritious meals tailored to individual dietary needs and assist with feeding or medication administration following care plans and HIPAA regulations. Support clients with medication management, ensuring proper administration and adherence to prescribed schedules. Provide transportation assistance for medical appointments, errands, or community activities while maintaining client safety and comfort. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and ensuring infection control standards are met. Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and update on patient progress or concerns related to dementia care, Alzheimer's care, or neurodevelopmental disorders. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients carefully and respond appropriately to changes in condition or behavior. Knowledge of HIPAA privacy regulations and experience maintaining confidentiality of patient information. Certification as a CNA or PCA is required; additional certifications such as CPR/First Aid are strongly recommended. Familiarity with specialized care techniques including Hoyer lifts, patient transfer methods, and managing residents' rights is essential. Experience supporting individuals with developmental disabilities, intellectual disabilities, autism spectrum disorder (ASD), dementia or memory impairments is advantageous. Ability to lift heavy objects safely and perform manual handling tasks while adhering to safety protocols.
