Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and mobility support using manual handling and patient transfer methods such as Hoyer lifts. Monitor patients' health status by observing vital signs, patient personal care needs, and overall condition; report changes promptly to healthcare professionals. Support medication administration in accordance with care plans and HIPAA regulations, ensuring accurate documentation and safety protocols are followed. Prepare nutritious meals tailored to individual dietary requirements and assist with meal consumption when necessary. Maintain a clean and safe environment through regular cleaning duties and adherence to infection control standards. Provide transportation assistance for appointments, errands, or community outings using appropriate client transportation techniques. Document all care activities thoroughly in compliance with resident rights, privacy laws, and facility policies while maintaining clear communication with clients and team members. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong patient observation skills with the ability to monitor vital signs and recognize signs of distress or changes in condition. Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism or intellectual disabilities. Familiarity with care plans, medication administration procedures, HIPAA compliance, and patient monitoring techniques. Ability to perform manual handling tasks including heavy lifting and patient transfers safely using equipment like Hoyer lifts. Excellent communication skills for engaging effectively with clients, families, and healthcare teams.
