Job Summary We are seeking a compassionate and dedicated Personal Care Assistant Caregiver to provide exceptional support and assistance to individuals in need of daily living care. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts when necessary. Monitor and observe patients' conditions closely, documenting vital signs, behavioral changes, and overall health status in compliance with HIPAA regulations. Prepare nutritious meals aligned with dietary plans, ensuring proper food safety and cleanliness standards are maintained. Support medication administration under supervision or as trained, following care plans meticulously. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed consistently. Engage residents in social activities and provide companionship to promote mental stimulation and emotional well-being. Collaborate with healthcare professionals to develop, implement, and update individualized care plans tailored to each resident's needs. Assist with transportation to medical appointments or community outings as required. Experience Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care settings is highly preferred. Demonstrated knowledge of patient observation techniques, vital signs monitoring, medication administration, and dementia or Alzheimer's care. Hands-on experience with patient care tasks such as assisting with ADLs, mobility support using Hoyer lifts or other assistive devices, and managing residents with developmental disabilities or autism. Familiarity with HIPAA regulations ensuring confidentiality of patient information at all times. Experience working with individuals requiring memory care or hospice support is advantageous. Ability to perform heavy lifting safely when assisting residents with mobility or transfers using appropriate equipment like Hoyer lifts.
