Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene. Support patients with mobility using proper manual handling techniques and equipment like Hoyer lifts or patient transfer methods. Monitor patient health by observing vital signs, noting changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Administer medications following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality. Provide transportation assistance for appointments or errands, ensuring safe and timely client transfers. Maintain a clean, safe environment by performing light housekeeping duties such as cleaning patient areas and ensuring safety protocols are followed. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly valued. Strong patient observation skills to identify changes in condition or behavior promptly. Knowledge of dementia care, Alzheimer's care, memory care, and hospice care best practices. Ability to assist with complex patient needs including medication administration, vital signs monitoring, and resident's rights adherence. Familiarity with assisted living environments, nursing homes, home health settings, and long-term care facilities. Competence in using patient transfer methods like Hoyer lifts and understanding manual handling safety procedures. Excellent communication skills for effective client interaction and teamwork within multidisciplinary healthcare teams.
