Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper manual handling and patient transfer methods such as Hoyer lifts when necessary. Monitor patients' vital signs and observe for any changes in health status, ensuring timely communication with healthcare teams. Support medication administration following HIPAA guidelines and care plans to ensure safe and accurate delivery of treatments. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Provide transportation assistance for appointments, errands, or community outings to enhance clients' independence. Maintain detailed documentation of care provided, patient observations, and adherence to resident's rights policies. Uphold safety standards by practicing first aid and CPR when needed, managing heavy lifting safely, and ensuring a clean and organized environment through regular cleaning duties. Skills Proven experience working with individuals with disabilities, including developmental disabilities, neurodevelopmental disorders such as autism or dementia care. Strong knowledge of patient monitoring techniques, care plans, and medication administration procedures. Familiarity with assisted living environments, nursing homes, hospice care, or home health settings. Ability to perform manual handling tasks safely using equipment like Hoyer lifts and patient transfer methods; capable of heavy lifting when required. Excellent communication skills for client interaction and collaboration with healthcare teams while respecting residents' rights. Knowledge of HIPAA regulations to maintain confidentiality and privacy of patient information.
