Key Responsibilities:

Support individuals in developing independent living skills such as budgeting, cooking, and household management Implement and follow Individualized Support Plans (ISPs) and behavior support plans Assist with and/or provide transportation to appointments, work sites, and community activities Encourage and facilitate community integration and social engagement Provide on-site job coaching at community employer sites assisting individuals to explore their employment interests, develop needed skills, job shadow, and ultimately support them during training and to maintain their chosen employment Provide support to the Employment Consultant for Vocational Rehabilitation Participants Enter time daily and complete notes and required documentation within 48 hours of service Monitor and document individuals' progress, behaviors, and any incidents in a timely and accurate manner Maintain a safe, clean, and supportive environment Communicate effectively with team members, supervisors, families, and healthcare providers Attend all quarterly meetings to review and update ISP Respond appropriately to emergencies and follow established protocols

Qualifications:

High school diploma or GED required Must be at least 18 years old Valid driver's license and reliable transportation (preferred or required depending on employer) Proof of vehicle insurance Ability to pass background checks, including Indiana criminal history and abuse registry checks CPR/First Aid certification (or willingness to obtain) Previous experience in caregiving, human services, or working with individuals with disabilities preferred

Work Environment & Schedule:
