We are seeking compassionate, dependable, and patient-focused Home Health Aides (HHA) and Certified Nursing Assistants (CNA) to join our growing team on a Part-Time/PRN basis . Competitive pay Flexible scheduling One-on-one patient care Supportive nursing and clinical leadership Consistent patient assignments when available 401(k) retirement plan Fast and streamlined hiring process Meaningful work that makes a difference every day Job Responsibilities As a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), you will: Provide personal care services in patients' homes according to the established plan of care Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility Support patient safety, comfort, and independence Observe, document, and report changes in patient condition to the supervising nurse or clinical team Assist with transfers, ambulation, and mobility support Provide medication reminders and other health-related tasks as permitted by state regulations and agency policy Maintain a clean, safe, and healthy home environment Educate family members and caregivers on appropriate care techniques when applicable Follow infection control, safety, and confidentiality standards Comply with all agency policies, procedures, and Medicare home health guidelines Qualifications Required Current Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certification (as applicable) Valid driver's license, reliable transportation, and auto insurance Ability to travel throughout Lake County and Porter County, Indiana Strong verbal and written communication skills Ability to work independently in a home care environment Ability to perform physical caregiving duties, including lifting, transferring, and assisting patients with mobility Preferred Previous experience in home health, hospice, private duty, assisted living, long-term care, or skilled nursing Experience providing personal care and ADL assistance Knowledge of patient safety and infection prevention practices
