In this vital role, you will provide attentive assistance to residents or clients in various settings such as assisted living facilities, nursing homes, or home health environments. Responsibilities Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient conditions through observation of vital signs, behavior, and overall health status; report changes promptly to healthcare professionals. Support medication administration following care plans and HIPAA regulations to ensure privacy and safety. Prepare nutritious meals and assist with meal consumption, ensuring dietary needs are met. Use proper manual handling techniques and patient transfer methods such as Hoyer lifts or other assistive devices to ensure safety during movement. Maintain cleanliness of personal spaces and common areas by performing light cleaning tasks in accordance with health standards. Document care activities accurately in client records and communicate effectively with team members regarding patient progress or concerns. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, vital signs measurement, medication administration, and basic first aid/CPR certification. Experience working with individuals with dementia, Alzheimer's disease, autism spectrum disorder, developmental disabilities, or neurodevelopmental disorders is advantageous. Familiarity with HIPAA regulations and maintaining confidentiality of patient information. Ability to perform heavy lifting and manual handling safely; comfortable assisting with transfers using equipment like Hoyer lifts. Strong communication skills for effective client interaction and teamwork within multidisciplinary care settings. Valid certifications in first aid and CPR; additional training in hospice care or social work is a plus. Demonstrated ability to provide compassionate patient personal care assistance while supporting independence and dignity.
