Job Description
Hobart Medical Building 101 W 61st Ave Hobart, Indiana 46342 The Home Health Hospice Aide is a valuable position among the Franciscan team. This position actively participates in the delivery of high-quality patient care either in the home or inpatient setting. The Home Health Hospice Aide works under the supervision of the Franciscan Health Licensed Clinician.

Monday to Friday- 8am to 5pm
REQUIRED
Active CNA or HHA License with the State of Indiana. Must have valid Driver's License and clear driving record. Assist patients with ambulation and transfer using proper body mechanics. Communicate any change in condition or lack of patient progress. Complete necessary documentation of care provided. Record intake, output, and vital signs as directed and communicate care, as appropriate. Provide assistance with personal hygiene and elimination needs, according to the established plan of care. Schedule visits with the patient or caregiver. Assist patients with care and activities of daily living, as directed in the established Plan of Care.QUALIFICATIONS
High School Diploma/GED- Required 1 Year Long Term Care Setting Experience
- Preferred 1 Year Home Healthcare Experience
- Preferred 1 Year Hospice Experience
- Preferred Certified Nursing Assistant (CNA) OR Home Health Aide (HHA
