Job Overview Join our dedicated healthcare team as a Home Health Care Aide, where your compassionate care and proactive support will make a meaningful difference in the lives of patients receiving home health services. In this vital role, you will assist individuals with daily activities, monitor their health status, and provide personalized care in a safe and respectful manner. Your energetic approach and attention to detail will help promote independence, comfort, and well-being for our clients while adhering to all home healthcare regulations and standards. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, respiration, and temperature, ensuring timely reporting of any changes to healthcare professionals. Support medication administration following prescribed instructions while maintaining strict compliance with HIPAA and patient confidentiality protocols. Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques and behavioral management. Document medical records and care notes using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR), ensuring accuracy and completeness of medical documentation. Assist with medical procedures such as catheter care or wound management under supervision or according to training. Support patients during transfers or mobility exercises using proper patient transfer methods to ensure safety for both patient and caregiver. Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met. Qualifications Proven experience working in assisted living facilities, nursing homes, or home healthcare settings with geriatric or disabled populations. Knowledge of home health care regulations, medical terminology, and best practices for patient safety. Certification in First Aid and Basic Life Support (BLS), including CPR training. Ability to work independently while maintaining excellent communication with clients and healthcare teams. Experience working with individuals with developmental disabilities or end-of-life care needs is highly desirable. Familiarity with EMR/EHR systems for medical documentation and client records management. Physical ability to perform heavy lifting when necessary and assist with patient transfers safely using appropriate equipment like Hoyer lifts or patient transfer techniques.
