Job Description

Job Summary Join our compassionate team as a Home Health Aide, where your dedication will make a meaningful difference in the lives of individuals needing personalized care. In this vital role, you will provide essential support to patients in their homes and community settings, promoting independence, comfort, and well-being. Your active engagement and attentive care will help clients maintain their dignity and quality of life while ensuring their safety and health are prioritized every day. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts and wheelchairs and gate belts Support patients with specialized needs such as dementia, Alzheimer's disease, memory care, or developmental disabilities through tailored caregiving strategies. Maintain detailed medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure accurate documentation of care provided. Prepare nutritious meals and assist with feeding, ensuring dietary restrictions are followed precisely. Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall health and comfort. Collaborate with healthcare professionals to coordinate care plans and update medical documentation as needed for hospice or long-term care situations. Experience Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments is highly preferred. Demonstrated ability to work with individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease. Certification in
