Job Description

Under supervision of designated supervisor, assume primary responsibility for individuals receiving day habilitation services in an active environment requiring physical movement.
Essential Duties and Responsibilities:
Regular and reliable attendance is necessary due to the nature of our work with disabled individuals and the regulations of the Commonwealth of Massachusetts, which requires minimum staffing levels. Hands on direct care (including personal care) with adults with developmental disabilities and/or medical challenges Assist in the execution of the program environment Assist in monitoring individual programs and support systems under the guidance of the Supervisory Staff Implement goal oriented and varied programming in a supportive setting with a team of dedicated professionals and provide daily instruction and support in Life skills, including personal care, self help and dining assistance Communication skills Socialization, recreation and community skills Independent living development Affective development/self advocacy and human rights Wellness development Case management responsibilities involve daily documentation, and monthly reports and giving feedback to our interdisciplinary team and assisting in providing all necessary information required for meetings Participate as a member of program team in preparation of activities, curriculum and program objectives Professional communication within the program as well as stakeholders such as families, residential vendors, etc. Participate in ongoing training and certifications specific to role requirements Perform such other tasks and assumes such other responsibilities as may be assigned by the Supervisory Staff Very active environment requiring a lot of movement
Knowledge, Skills and Abilities :
Experience working with individuals with developmental disabilities a plus but not required Willingness to learn and implement new philosophies and methods Excellent oral and written communication skills, adaptability and team-work skills required Proficiency in English is required
Minimum Acceptable Qualifications:
High School Graduation or G.E.D equivalent Ability to lift 30 pounds Driving requirement: A valid driver's license and satisfactory driving record are required for positions involving regular agency driving. Employees assigned to regularly transport Day Habilitation participants must possess a valid Massachusetts driver's license and meet all applicable regulatory, contractual and automobile-insurance requirements. The requirement for a Massachusetts driver's license may be overridden for purposes of evacuations of our Day Habilitation participants during emergency situations. CORI check and National Background check required (to be completed upon hire)

While performing the duties of this job, the employee is frequently required to push wheelchairs. The employee must be able to lift and/or move up to 30 pounds.
