Job Description

NFI Massachusetts Inc, a non-profit human services agency, is currently seeking two (2) Case Managers for its DDS Residential programs specializing in adults with Acquired Brain Injuries (ABI) located in Methuen, MA and Haverhill, MA. These programs offer residents a full range of services including assistance with activities of daily living, recreational opportunities, individual support, transportation, and coordination of medical needs and services. The goal of the program is to enable adults to live in a least restrictive setting, to offer the maximum level of independence, and to fully engage in community life. The Case Manager will be responsible for: Maintain required staffing ratios and provide direct supervision and support to individuals served. Assist individuals with activities of daily living (ADLs), personal care, mobility, and other support needs as identified in their individual service plans. Provide support and supervision during community activities and appointments. Accompany and support individuals at medical, dental, therapy, and other healthcare appointments as assigned. Communicate relevant observations, changes in condition, and follow-up needs to appropriate program and clinical staff. Follow all applicable health, safety, medication, documentation, and program requirements. Promote the dignity, independence, choice, and overall well-being of individuals served. Complete administrative tasks delegated by Program Management. Assist with program documentation, filing, data entry, scheduling, record organization, and other administrative duties as assigned. Support Program Management with program-related projects and tasks as needed. Maintain confidentiality and comply with agency policies and applicable regulations.
Job Requirements:
Must have 3 plus years of experience working with individuals with Acquired Brain Injuries. Bachelor's degree in Psychology, Social work or a related field preferred but not required. Valid driver's license and a reliable vehicle are essential
Salary :
