About the Role The Residential Case Manager is an advanced direct support professional who combines hands-on support with case management responsibilities to help adults with intellectual and developmental disabilities thrive in community-based residential settings. Working under the direction of the Program Manager, you'll support individuals in achieving their personal goals, maintaining their health and well-being, strengthening community connections, and increasing independence. Approximately 75% of this role involves direct support services, while 25% focuses on case management, healthcare coordination, advocacy, and service planning. What You'll Do Direct Support & Individual Care Provide compassionate, person-centered support in daily living activities. Assist with personal care, meal preparation, household tasks, community participation, and recreational activities. Promote independence, self-determination, dignity, and personal choice. Support individuals in building and maintaining relationships with family, friends, and community members. Assist with money management and other independent living skills. Case Management & Service Coordination Coordinate medical, dental, behavioral health, and other support services. Schedule and track appointments and ensure appropriate follow-up care. Accompany individuals to appointments as needed. Communicate with healthcare providers, family members, guardians, service coordinators, and support teams. Assist individuals in accessing benefits, community resources, and needed services. Advocate for individuals to ensure their needs and goals are addressed. ISP & Documentation Responsibilities Implement and monitor Individual Service Plan (ISP) goals and objectives. Participate in ISP meetings and provide input regarding progress and support needs. Complete progress notes, appointment tracking, monthly summaries, and required documentation accurately and timely. Maintain compliance with DDS, licensing, accreditation, and agency requirements. Health & Safety Administer medications in accordance with MAP regulations. Monitor health and well-being and report concerns appropriately. Respond to emergencies and complete required incident reporting. Help maintain a safe, clean, and welcoming residential environment. Program Support Utilize approved flex time for case management activities, appointments, ISP meetings, documentation, and service coordination. Participate in on-call coverage as assigned. Attend trainings, meetings, and professional development activities. What We're Looking For Required Qualifications High School Diploma or GED. Minimum one year of experience supporting individuals with intellectual and developmental disabilities, behavioral health needs, or related populations. Valid driver's license with a satisfactory driving record. Ability to transport individuals using agency vehicles. Ability to obtain and maintain MAP, CPR, First Aid, and other required certifications. Strong communication, organizational, and problem-solving skills. Basic proficiency with Microsoft Office, email, and electronic documentation systems. Ability to work independently while collaborating as part of a team. Preferred Qualifications One year of experience as a Residential Counselor II or equivalent advanced direct support role. Associate's or bachelor's degree in human services, Social Work, Psychology, Special Education, Healthcare, or a related field. Experience coordinating medical appointments and community services. Knowledge of DDS regulations, ISP implementation, and person-centered planning.
