Community Connections Inc. is looking for Direct Support Professionals to join our Day Hab team based in Yarmouth, MA. In this role, you will support individuals with disabilities by implementing their Day Habilitation Service Plan (DHSP) and documenting their progress. You will help people become more engaged and reach their maximum potential in physical, emotional and social abilities. You will partner with Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. Direct Support Professional:
Implement Day Habilitation Service Plan (DHSP) objectives Document each person's progress towards DHSP objectives Assist individuals physically as necessary Ensure individuals health, safety and general well being Provide and maintain community based habilitation/therapeutic experiences through social, recreational and volunteer opportunities, based on the individual's needs and wishes Ensure and safeguard individuals' human dignity, rights, and liberties What You'll Need as a
Direct Support Professional:
High School Diploma or General Education Degree (GED) 1+ year's experience and/or training working with individuals with disabilities Valid driver's license and satisfactory driving record Proven problem solving abilities Excellent written and verbal communication and interpersonal skills Demonstrated ability to work independently Strong computer and word processing skills
