Duties Assist residents with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately while adhering to HIPAA privacy regulations Prepare nutritious meals and assist with meal planning and preparation to meet individual dietary needs Support residents during medication administration in compliance with care plans and safety protocols Provide companionship and engage residents in recreational activities to promote mental well-being and social interaction Maintain a clean, safe environment through regular cleaning duties and proper sanitation practices in accordance with health standards Assist with transportation for appointments or community outings using personal transportation assistance or agency vehicles when necessary Follow resident's rights policies, ensuring dignity, respect, and personalized care tailored to each individual's preferences and needs Document care activities accurately in electronic health records or paper logs, including patient monitoring data, incident reports, and updates on care plans Participate in ongoing training related to dementia care, Alzheimer's care, autism support, neurodevelopmental disorders, hospice care, or other specialized areas as needed Experience Prior experience working in group homes or home environments preferred Demonstrated ability to provide patient personal care assistance with compassion and professionalism Knowledge of HIPAA regulations and confidentiality standards related to patient information Hands-on experience with patient monitoring tools Familiarity with medication administration procedures following prescribed care plans Experience working with individuals with developmental disabilities or neurodevelopmental disorders such as autism or intellectual disabilities support is highly valued Strong communication skills for effective client interaction and teamwork within multidisciplinary settings Certifications in CPR and First Aid are required; Ability to perform heavy lifting safely and conduct manual handling tasks regularly while maintaining proper body mechanics Understanding of client's rights policies and commitment to promoting independence and dignity for all residents Must have a car and proof of auto insurance Join us as a Direct Support Professional where your dedication transforms lives!
