Overview Evergreen Healthcare Solutions Inc. is seeking a dependable and compassionate DSP to provide person-centered Community Development Services to an individual in Hagerstown. The DSP will promote independence, community inclusion, skill development, and progress toward goals identified in the individual's Person-Centered Plan (PCP) and Personal Support to an individual in Knoxville Maryland.
Qualifications High school diploma or GED. Valid driver's license, satisfactory driving record, and reliable insured vehicle. Ability to pass required background and registry checks. Ability to complete all required DDA and agency training. Comfortable using a smartphone or computer for documentation and timekeeping. Must be dependable, punctual, patient, respectful, and able to work independently. Previous experience supporting individuals with intellectual or developmental disabilities is preferred. Must be authorized to work in the U.S. Strong communication, patience, reliability, and a compassionate attitude. Knoxville , Maryland 21740Schedule:
Full-time, 40 hours per week Responsibilities Provide support based on the individual's PCP, needs, preferences, and goals. Assist with community activities, appointments, volunteering, and recreational opportunities. Assist with daily-living and personal support needs while encouraging maximum independence. Promote communication, social, decision-making, and independent-living skills. Encourage informed choices and active participation in daily planning. Provide safe transportation and monitor the individual's health and safety. Follow applicable behavioral, health, and safety support plans. Complete progress notes, attendance records, incident reports, and mileage logs accurately and on time. Maintain confidentiality and professional communication with the support team. Assist with meal preparation, household routines, personal care, and medication support when authorized and properly trained. Report incidents, concerns, injuries, or changes in condition promptly.
