Job Description

Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. This vital role involves assisting individuals with daily activities, ensuring their safety, and promoting independence within the comfort of their homes. The ideal candidate will be energetic, detail-oriented, and committed to delivering personalized care that enhances quality of life. As a Home Health Aide, you will play a crucial part in supporting clients' physical, emotional, and social well-being while adhering to healthcare regulations and maintaining strict confidentiality. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and observe patients' health status by taking vital signs, recording medical information accurately in Electronic Health Record (EHR) systems, and reporting any changes to healthcare professionals promptly. Provide medication administration assistance following prescribed schedules and medical terminology guidelines while ensuring compliance with home healthcare regulations. Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and end-of-life support with patience and empathy. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Maintain patient confidentiality in accordance with HIPAA regulations while documenting medical records and care activities thoroughly. Use patient transfer techniques safely during patient care activities and assist with mobility using appropriate equipment like Hoyer lifts or patient transfer methods. Communicate effectively with clients and their families to understand needs and provide emotional support within the scope of caregiving responsibilities. Follow all safety protocols including first aid, CPR certification requirements, and basic life support procedures to ensure client safety at all times. Skills Proven experience working with individuals with disabilities or in assisted living environments; experience in nursing homes or geriatrics is highly valued. Familiarity with home health agency operations, home & community care standards, and home health regulations. Proficiency in using EMR/EHR systems for accurate documentation of patient information and medical records. Excellent patient observation skills combined with compassionate caregiving abilities tailored for geriatric patients or those requiring hospice or end-of-life care. Ability to perform heavy lifting when necessary while maintaining safety protocols; experience with equipment like Hoyer lifts is preferred. Effective client communication skills paired with the ability to work independently or as part of a team delivering home health care assistance. Knowledge of HIPAA compliance standards to maintain patient confidentiality at all times; experience working within home health or long-term care settings is advantageous.
