Medical Social Worker MSW PRN Home Health Make a Difference Beyond the Clinical Visit A safe recovery at home often depends on more than medical care. Patients may also need emotional support, financial resources, transportation, housing assistance, or help navigating community programs. Elara Caring is hiring a PRN Medical Social Worker to support home health patients and families throughout Portland and surrounding southern Maine communities. You will work independently in the community while receiving support from an experienced interdisciplinary team. One-on-one support in the patient's home Meaningful relationships with patients and families Supportive interdisciplinary clinical team Independence and flexibility in the field Continuing education opportunities Tuition reimbursement for eligible employees Opportunities for professional growth 401(k) with company match Medical, dental, vision, paid time off, and additional benefits based on eligibility What You'll Do Assess patients' psychosocial, financial, and environmental needs Complete initial social work evaluations and follow-up visits Provide counseling, education, and emotional support to patients and caregivers Connect patients and families with community programs and financial resources Identify barriers that may affect a patient's safety or ability to remain at home Participate in developing and updating individualized Plans of Care Collaborate with physicians, nurses, therapists, and other care team members Support discharge planning and continuity of care Communicate changes or concerns affecting patient safety and well-being Complete accurate and timely clinical documentation What You'll Bring Master's or Doctoral degree in Social Work from a program accredited by the Council on Social Work Education Current Maine Social Worker license in good standing At least one year of social work experience in a healthcare setting Strong communication, counseling, documentation, and advocacy skills Ability to work independently in patients' homes Willingness to travel throughout the assigned service area with 50% travel required Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations Ready to Make an Impact?
