Job Summary (3rd shift) Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your energetic approach and commitment to quality patient care will help promote independence and comfort for those in need of home healthcare services. This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or end-of-life care. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems Support medication administration following medical instructions and maintaining strict adherence to home health regulations and HIPAA confidentiality standards Observe patient conditions closely, noting any changes or concerns related to health status or behavior for timely reporting Perform patient transfers using safe methods such as Hoyer lifts or transfer belts while ensuring safety protocols are followed Provide specialized care including Alzheimer's care, dementia support, memory care, and end-of-life assistance with compassion and professionalism Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, and meal preparation in accordance with client needs Skills Strong knowledge of medical terminology and documentation practices for accurate record keeping Experience working with geriatric patients, individuals with developmental disabilities or disabilities requiring specialized caregiving Proficiency in using EMR (Electronic Medical Records) systems and home health agency software for documentation and communication Ability to perform patient personal care assistance with empathy while respecting patient dignity and confidentiality under HIPAA guidelines Excellent communication skills for effective client interaction and collaboration with healthcare teams Knowledge of CPR (Cardiopulmonary Resuscitation) and basic life support techniques for emergency situations Physical stamina for heavy lifting, patient transfers, and long periods of standing or walking during shifts Familiarity with home health care regulations, hospice care procedures, and long-term care best practices Join us if you're passionate about providing high-quality home healthcare assistance that truly transforms lives.
