House Coordinator
JOB SUMMARY
Staff member is primarily responsible for the day-to-day operations of providing hospice comfort care with dignity to end-of-life guests including, but not limited to guest care, dispensing medications, and record keeping of such actions in both daily log and medication log respectively. Perform daily hygiene for guests and their environment, provide nutritional needs of the guests. Provide emotional support to guests and all visitors. Maintain a close dialogue with the House Coordinator and individual hospice provider to ensure that all directives are followed as prescribed. Complete general housekeeping and maintenance as assigned. Sometimes this requires documenting.

These core principles focus on providing empathetic support to patients and families, relieving pain through symptom management, and maintaining transparent dialogue to ensure care aligns with the patient's wishes at the end-of-life.
JOB QUALIFICATIONS & REQUIREMENTS
Personal assistance services may be performed by an unlicensed person who is at least 18 years of age and has demonstrated competency, when competency cannot be determined through education and experience, to perform the tasks assigned by the supervisor. An unlicensed person who is under the age of 18, is a high school graduate, or is enrolled in a vocational educational program and has demonstrated competency to perform the tasks assigned by the supervisor, may perform personal assistance services. Be alert to "guests" needs at all times. Sometimes the verbal communication is gone so you look for signs of discomfort for evaluating needs and seek hospice agency directives. Ability to clearly explain to the providing hospice agency a change or decline of their patient. Good sensory perception to be able to listen carefully to weaken guest requests and good listening skills to sounds from at a distance. Work in teams. Each shift has two staff members on duty at all times. Good physical health with the ability to climb, balance, kneel, crouch, reach, walk, sit, lift, grasp. Be able to perform repetitive motion involving lifting, pushing, pulling, or carrying objects weighting over 50 lbs with assistance. Review daily documented guest logs at the change of shift. Ask questions for clarity. Review daily documented medication logs and audit medication intake. Ask questions for clarity and make note of any dosage allowance is over or under. Those MUST be documented and initialed by one staff member leaving and a incoming staff member Attend staff meetings and information sharing Staff evaluation and first 90-day reviews are completed, in person, by the employee and House Coordinator. The Board President if the House Coordinator is unavailable. Confidentiality is a must. Any release of information is a violation of HIPPA laws and our employee Confidentiality Signed Statement.
QUALIFICATIONS
Preferred two years of health, medical related experience within the last three years Preferred knowledge of hospice philosophy and principles Ability to work a flexible schedule, some weekends and holidays Strong written and verbal communication skills Must have transportation and a valid drivers license
INTERPERSONAL QUALIFICATIONS
A professional, positive attitude and exhibiting proper conduct, appearance, communication, mutual respect and ethical behavior. Must be able to maintain professionalism under stress with guests/families due to grief and loss. Must wear a professional attire of clean scrubs allowing for comfort, modesty, and ease of movement. Required closed-toe, non-skid shoes for safety, along with a employee ID. Brian's House has a printed staff shirt that can be worn to replace the scrub top. No jeans are allowed unless informed by the TITLE which will need approval of the Board of Brian's House.
PHYSICAL REQUIREMENTS
Ability to perform proper body mechanics with manual dexterity and mobility. Good physical health with the ability to climb, balance, kneel, crouch, reach, walk, sit, lift, grasp. Be able to perform repetitive motion involving lifting, pushing, pulling, or carrying objects weighting over 50 lbs with assistance. Good sensory perception to be able to listen carefully to weaken guest requests and good listening skills to sounds from at a distance.
POSITION
Hourly One week unpaid vacation after 90 days on the job State sick accrual allowance
HOLIDAY PAY
