You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities:

Assist clients with daily living activities, including bathing, grooming, and meal preparation. Provide companionship and emotional support to enhance clients' quality of life. Administer medication and monitor health status as directed by healthcare professionals. Help clients with mobility and transportation to appointments or social activities. Maintain a clean and safe environment for clients. Document and report changes in clients' conditions to the care team. Collaborate with family members and healthcare providers to ensure comprehensive care. Participate in training and development programs to enhance caregiving skills.

Requirements:

High school diploma or equivalent; additional caregiving certifications preferred. Previous experience in direct care or a related field is highly desirable. Valid driver's license and reliable transportation. Strong communication and interpersonal skills. Ability to lift and assist clients in a safe manner. Compassionate and patient demeanor with a passion for helping others. Willingness to work flexible hours, including weekends and holidays. Background check and professional reference checks required.
