D Square Healthcare Services Inc is looking for a compassionate and dedicated Direct Support Professional to join our team in Belle Plaine, MN. Responsibilities:

Provide direct care and daily living support to individuals with intellectual or developmental disabilities Assist clients with personal hygiene, grooming, meal preparation, and household tasks Implement individualized support plans (ISPs) and document progress accurately Transport and accompany clients to medical appointments, community activities, and social outings Administer medications and follow health care protocols as directed Foster independence, dignity, and community inclusion for each individual served Communicate effectively with supervisors, family members, and healthcare professionals

Requirements:

High school diploma or GED equivalent required Prior experience in direct support, caregiving, or human services preferred Valid driver's license and reliable transportation Ability to pass a background study through the Minnesota Department of Human Services (DHS) CPR and First Aid certification (or willingness to obtain) Strong communication, patience, and problem-solving skills Ability to work flexible shifts, including evenings, weekends, or overnights
