•7:00 AM About the Position Divine Care Spaces is looking for dependable, compassionate, and experienced caregivers to join our team at our assisted living residence in Brooklyn Park. As a Home Health Aide/Caregiver, you will provide hands-on support to residents with their daily needs while helping maintain a safe, comfortable, and respectful home environment. We are looking for caregivers who are patient, responsible, attentive, and able to work independently while following each resident's individualized care plan. Responsibilities Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and mobility Assist residents with transfers, repositioning, and safe movement throughout the home Provide medication assistance or administration as directed and permitted by the resident's care plan and nursing staff Monitor and document resident condition, behaviors, vital signs, and changes in health status as required Promptly report health or safety concerns to the appropriate supervisor or nurse Prepare and serve meals and assist residents with eating when needed Perform light housekeeping duties, including laundry, changing bed linens, cleaning resident areas, and maintaining a clean living environment Provide companionship and engage residents in conversation, activities, walks, games, or other appropriate activities Assist residents with appointments, outings, and transportation when assigned Follow resident care plans, facility policies, infection-control procedures, and safety protocols Complete accurate shift documentation and communicate important information to incoming staff Respond appropriately to emergencies and changes in resident condition Maintain resident privacy, dignity, and confidentiality at all times Qualifications High school diploma or GED preferred Previous professional caregiving experience strongly preferred Experience in assisted living, home care, group homes, memory care, hospice, or a similar healthcare setting is preferred Experience assisting with medication administration and monitoring vital signs is a plus Must be comfortable providing hands-on personal care Must be dependable and able to work scheduled 12-hour shifts Ability to communicate professionally with residents, families, coworkers, nurses, and supervisors Ability to complete required documentation accurately Valid driver's license preferred/required for positions involving resident transportation CPR/First Aid certification preferred; must be willing to obtain if required Must be able to successfully complete all required background checks and employment requirements Ideal Candidate We are especially interested in caregivers who: Have at least one year of professional caregiving experience Are dependable and consistently arrive to work on time Have a reliable mode of transportation Are comfortable working independently Have experience supporting adults who need assistance with daily living Can remain calm and professional during difficult or unexpected situations Take initiative and do not need constant supervision to complete routine caregiving duties Treat residents with patience, dignity, and respect Benefits Flexible scheduling Earned Sick and Safe Time (ESST) in accordance with Minnesota law Employee referral program Consistent 12-hour shifts Supportive residential work environment Apply If you have caregiving experience and are looking for a position where you can build meaningful relationships with residents while providing high-quality care, we encourage you to apply.
