Job description Epiphany Care Homes was created for seniors with dementia, Alzheimer's, and mood disorders. We focus on providing 24-hour compassionate assisted living support in a residential home environment. We have four residential assisted living facilities in Minnetonka and Golden Valley. The ultimate goal is to improve the residents' quality of life by assisting with their health care needs, behavior management, and personal goals with utmost compassion and patience. Working in a residential home, ideal candidates will be able to provide cares, administer medications, communicate to the nurse, interact with residents and be comfortable in the kitchen preparing meals. At this facility, there is either a 1:7 CNA staffing ratio, with only one CNA scheduled per shift. Home Health Aide/PCA will work 4-hour shifts alongside CNA Staff. Home Health Aide/PCA shifts will be scheduled from 6 AM

• 10 AM, and 4 PM
• 8 PM. Home Health Aides/PCAs on shift will have additional expectations beyond care administration that include but are not limited to food preparation, house keeping, and activities with residents. All interviews will be set up through Indeed.com. Starting base pay $17
• $19 an hour: based on experience.

Requirements:

• Home Health Aide/PCA Certification
• Demonstrate ability to read, write, speak, and understand the English language to communicate clearly with all residents, staff, and family
• Punctual, Responsible, Compassionate, and Kind
• Must have reliable transportation to and from work
• Able to pass a background check
• Be willing to work with seniors with cognitive and mental health issues
• Demonstrate ability to use computer charting in PointClickCare
• Demonstrate ability to use scheduling application SchedulePop and ProLiant to track hours, pick up shifts, and monitor schedule
• Demonstrate ability to prepare and serve meals, following provided recipes, following safe food handling procedures such as temperature checks, food logs, and refrigerator organization.

• 6 AM
• 10 AM weekdays and every other weekend

• 8 PM weekdays and every other weekend
