Job Overview We are seeking a compassionate and dedicated Personal Care Assistant (PCA) / Certified Nursing Assistant (CNA) to provide high-quality, personalized care to individuals in various care settings. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Monitor patient conditions through diligent observation of vital signs and overall health status, reporting any concerns promptly. Support patients with medication administration following prescribed protocols while maintaining accurate medical documentation in Electronic Health Record (EHR) systems. Provide specialized care such as catheter management, dementia care, Alzheimer's and memory care, as well as hospice and end-of-life support. Utilize patient transfer methods safely using equipment like Hoyer lifts and other mobility aids to prevent injury. Maintain patient confidentiality in accordance with HIPAA regulations while ensuring compliance with home health care regulations. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Communicate effectively with clients and their families to address needs and provide emotional support during caregiving activities. Experience Prior experience working with geriatric patients or in assisted living facilities is highly desirable. Demonstrated knowledge of medical terminology, home health care assistance, and medical documentation practices. Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving is preferred. Familiarity with EMR/EHR systems for maintaining accurate patient records and medical histories. Certification in First Aid and Basic Life Support (BLS), including CPR training, is required or will be provided upon hiring. Previous employment in nursing homes or home health agencies that involved patient transfer methods such as Hoyer lifts is advantageous. Knowledge of home & community care regulations along with experience in hospice or end-of-life care settings enhances suitability for this role.
