Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper manual handling and patient transfer methods such as Hoyer lifts. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately to nursing staff. Medication reminders following HIPAA guidelines and care plans established by healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition. Maintain a clean and safe environment by performing regular cleaning duties, adhering to infection control protocols, and ensuring resident spaces are hygienic. Provide personal transportation assistance for appointments or community outings, ensuring safety and comfort at all times. Document patient observations, care provided, and any incidents diligently to ensure compliance with healthcare standards and resident rights. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and basic first aid/CPR certification. Experience working with individuals with dementia, Alzheimer's disease, autism spectrum disorder (ASD), developmental disabilities or neurodevelopmental disorders is advantageous. Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information. Ability to perform heavy lifting safely and use manual handling equipment such as Hoyer lifts for patient transfers. Excellent communication skills for engaging with residents, families, and healthcare team members effectively. Demonstrated compassion for geriatrics, hospice care, memory care, or long-term care populations is essential.
