Job Overview We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA) to join our dynamic healthcare team. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility assistance using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs including blood pressure, pulse, respiration, and temperature; report changes promptly to nursing staff. Support medication administration following proper medical documentation protocols while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality. Provide specialized care for individuals with Alzheimer's disease, dementia, or other memory impairments through tailored memory care techniques and compassionate engagement. Document all patient interactions accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; ensure compliance with home health care regulations. Assist with medical procedures such as catheter care and wound management under supervision; perform basic first aid in emergency situations. Support patients with end-of-life or hospice care needs with sensitivity and professionalism; assist with meal preparation, light cleaning, laundry, and other home & community care tasks as needed. Qualifications Valid certification as a Certified Nursing Assistant (CNA) or Personal Care Assistant (PCA). Proven experience working in assisted living facilities, nursing homes, or home healthcare environments. Strong knowledge of medical terminology, patient care techniques, and medical documentation standards. Experience working with individuals with disabilities or developmental delays is highly desirable. Familiarity with EMR/EHR systems and home health agency procedures. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Excellent communication skills for effective client interaction and team collaboration. Knowledge of home health care regulations, HIPAA compliance, and safety protocols including first aid and basic life support (CPR).
