Monitor patients' vital signs, observe their condition closely, and report any changes or concerns to healthcare professionals promptly. Support medication administration following HIPAA guidelines and care plans developed by medical teams. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking. Provide personal transportation assistance for appointments, errands, or community outings to promote independence and social engagement. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and maintaining resident spaces according to health standards. Document patient observations accurately, update care plans regularly, and ensure residents' rights are upheld at all times. Requirements Proven experience working with individuals with disabilities, neurodevelopmental disorders, dementia, Alzheimer's disease, or in senior care settings such as nursing homes or assisted living facilities. Knowledge of patient observation techniques, medication administration, first aid, CPR certification, and basic life support skills. Familiarity with HIPAA regulations to ensure confidentiality of patient information. Ability to perform heavy lifting safely and use manual handling equipment like Hoyer lifts effectively. Strong communication skills to interact empathetically with clients, families, and healthcare teams. Experience working with residents who have memory care needs or require specialized dementia or Alzheimer's care is highly desirable.
