Consumers' Home Position Overview Support two individuals with Intellectual and Developmental Disabilities (IDD) as a live-in employee (24/7). The position involves extensive responsibilities that require a high level of skill, commitment, and accountability to ensure the health, safety, and overall well-being of the individuals served. We are seeking someone who is compassionate, motivated, and reliable. One individual also has a Behavior Support Plan and receives ABA services. While capable of living independently with appropriate supports, the individual requires consistent guidance and assistance in developing coping skills and managing emotional responses when upset. More detail will be provided during the interview. Key Responsibilities Assistance with Activities of Daily Living (ADLs), including bathing, showering, grooming, dressing, and toileting. Preparation of and assistance with three nutritious meals daily (breakfast, lunch, and dinner). Coordination with medical providers, guardians, Managed Care Organizations (MCOs), MCO monitors, Qualified Professionals (QPs), therapists, social workers, and other specialists involved in the individuals' care. Medication administration and medication education. Transportation to medical appointments, community activities, and other necessary destinations. Light housekeeping duties, including cleaning, laundry, and maintaining a safe, organized, and clutter-free environment. Provide person-centered support to individuals with IDD, focusing on skill development, community engagement, and independence. Promote personal growth and confidence by encouraging participation in daily routines and life skills. Collaborate and communicate effectively with the care team to ensure continuity and quality of care. Qualifications Ability to Lift/Push/Pull 25 pounds unassisted on a regular basis and up to 100 pounds occasionally 3+ Years of Unsupervised Driving Experience Comfortable providing personal care (dressing, showering, grooming, toileting, etc) Experience supporting individuals with intellectual and developmental disabilities (IDD) required Experience working with individuals with mental health High school diploma or GED minimum required Strong compassion, patience, and reliability Excellent communication and interpersonal skills Dedication to promoting independence, inclusion, and dignity for every individual Why Join Us At A Caring Heart Case Management, Inc., we believe that every individual deserves the opportunity to learn, grow, and thrive.
