Responsibilities Deliver personal care assistance including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts or other equipment. Monitor patient health by taking vital signs, observing patient conditions, and accurately documenting medical information using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Assist with Activities of Daily Living (ADLs), ensuring patients' safety and comfort while maintaining their dignity. Administer medications following home health care regulations and medical documentation protocols, including medication administration and catheter care. Support patients with memory care needs such as Alzheimer's or dementia by implementing specialized techniques in a safe environment. Maintain patient confidentiality in accordance with HIPAA regulations while communicating effectively with clients, families, and healthcare teams. Perform light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Respond promptly to emergencies by applying first aid or basic life support (BLS) techniques when needed. Assist with patient transfers using proper methods to prevent injury for both the patient and caregiver. Support hospice or end-of-life care efforts with compassion and professionalism while respecting patient wishes. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred. Demonstrated experience working with individuals with developmental disabilities or special needs such as Alzheimer's or dementia is advantageous. Familiarity with medical terminology, medical records management, and documentation procedures is essential. Experience operating EMR/EHR systems for accurate record keeping and communication within healthcare teams. Knowledge of home health care regulations and best practices for senior care and geriatrics is beneficial. Certification in CPR/BLS and first aid is required; additional certifications such as hospice or heavy lifting training are a plus. Proven ability to handle heavy lifting safely using appropriate equipment like Hoyer lifts when necessary. Strong communication skills to effectively engage with patients, families, and interdisciplinary teams while maintaining confidentiality under HIPAA guidelines.
