Job Description

Position Summary Provide individualized Community Networking supports that helps and individual with disabilities build community relationships, explore interests, increase participation, develop natural supports, and become more connected to community life in accordance with the authorized treatment plan. Reports To Qualified Professional (QP) / EHCS Designee Minimum Qualifications At least 18 years of age and able to meet EHCS and service-specific qualification requirements. Education, experience, screening, registry/credential, CPR/First Aid, and health documentation requirements completed as applicable to the assigned service. Ability to understand and implement the individual's ISP/person-centered plan and EHCS procedures. Ability to communicate respectfully and document services accurately and timely. Valid driver's license, acceptable driving status, and required insurance when transportation is an assigned duty. Essential Duties & Responsibilities Implement Community Networking goals, strategies, frequency, and authorized activities from the ISP/person-centered plan. Support the individual to identify and participate in community activities based on personal interests, preferences, strengths, culture, and goals. Facilitate relationship-building and natural supports rather than creating staff-dependent activities. Teach and reinforce community navigation, communication, social, safety, decision-making, and self-advocacy skills as identified in the plan. Support meaningful participation rather than simply accompanying or transporting the individual. Encourage choice and independence in planning activities, making purchases, interacting with others, and accessing community resources. Observe progress, barriers, risks, and changing support needs and communicate relevant information to the QP. Complete service documentation that identifies the community activity, goal addressed, supports provided, individual response/progress, and any follow-up needed. Provide transportation only when assigned/authorized and follow EHCS vehicle and passenger-safety requirements. Core EHCS Compliance Expectations Protect client rights, privacy, dignity, choice, cultural preferences, and confidentiality. Immediately report suspected abuse, neglect, exploitation, incidents, injuries, significant changes, and other health/safety concerns according to EHCS procedures. Maintain professional boundaries and comply with the EHCS Code of Conduct, gift limit, social media, privacy, and compliance requirements. Use person-centered, trauma-informed and least-restrictive approaches. EHCS does not use restraints or seclusion. Complete required orientation, ongoing training, supervision, and competency reviews. Documentation & Communication The DSP is responsible for complete, accurate, objective, person-centered documentation of services actually provided. Documentation must correspond to the authorized service, assigned goals/supports, actual time and location, and the individual's participation and response. Concerns that may affect health, safety, service delivery, authorization, or billing must be reported promptly.
Pay:
$16.00 - $20.00 per hour Expected hours: 20.0 - 40.0 per week
