Responsibilities Assist residents with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods like Hoyer lifts. Monitor patients' vital signs, observe changes in health status, and communicate findings to healthcare team members while adhering to HIPAA regulations for patient privacy. Administer medications accurately following prescribed care plans and document all medication administration activities diligently. Prepare nutritious meals and assist with meal planning to meet individual dietary needs while maintaining a clean and safe environment. Support residents during outings or appointments by providing personal transportation assistance and ensuring safety at all times. Engage in patient observation to identify signs of dementia, Alzheimer's disease, autism spectrum disorder, or other neurodevelopmental conditions; implement appropriate care strategies accordingly. Maintain a clean, safe living environment by performing routine cleaning tasks in accordance with health standards and facility policies. Follow all protocols related to resident rights, confidentiality (HIPAA), and safety procedures such as first aid and CPR certification requirements. Collaborate with team members to develop and update individualized care plans tailored to each resident's needs. Support residents' participation in community activities or recreational programs to promote social engagement and well-being. Experience Previous experience working in group homes, assisted living facilities, nursing homes, or home health environments is beneficial not mandatory. Demonstrated ability to provide patient care with patience, empathy, and professionalism. Experience working with individuals with developmental disabilities, intellectual disabilities, dementia, or neurodevelopmental disorders such as autism is advantageous. Familiarity with patient monitoring techniques including vital signs measurement and patient observation. Ability to perform light housekeeping tasks such as cleaning while maintaining compliance with health standards.
