Job Description

Job Summary We are seeking compassionate and dedicated Home Health Aides / Personal Care Assistants (PCA) to provide exceptional in-home care to clients with diverse needs. In this vital role, you will support individuals in maintaining their independence, comfort, and safety by assisting with daily activities and health management. Your positive attitude and attentive care will make a meaningful difference in the lives of those you serve. This position offers an opportunity to work closely with clients in their homes or community settings, promoting dignity and well-being every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status, recording vital signs and reporting any changes promptly Support clients with memory care needs such as Alzheimer's and dementia, ensuring a safe environment Maintain cleanliness of the client's home through light housekeeping tasks including laundry and cleaning Prepare nutritious meals tailored to dietary restrictions and assist with meal planning as needed Support clients with disabilities or those in assisted living or long-term care facilities to promote independence Requirements Strong patient observation skills to detect changes in health or behavior Experience working with individuals with developmental disabilities, dementia, or memory impairments Experience in Hospice , Dementia and Alzheimer's care preferred Excellent communication skills to collaborate effectively with clients, families, and healthcare teams Ability to perform light housekeeping tasks including cleaning and laundry efficiently Compassionate caregiving approach focused on patient dignity and comfort Experience working in home health settings or senior care environments is highly valued.
