Responsibilities Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts. Monitor patients' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly. Support medication administration following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality. Prepare nutritious meals and assist with meal planning to meet individual dietary needs, including specialized diets for dementia or Alzheimer's care. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control protocols are followed diligently. Document patient observations, care activities, and communication accurately in compliance with healthcare standards and resident's rights. Provide emotional support and companionship to residents or clients, fostering a positive atmosphere that encourages independence and dignity. Requirements Prior experience working in group homes, nursing homes, assisted living facilities, or home health settings is highly preferred. Knowledge of patient observation techniques, vital signs monitoring, medication administration, and basic first aid/CPR certification. Experience working with individuals with autism, neurodevelopmental disorders, dementia, or intellectual disabilities is advantageous. Ability to perform heavy lifting and manual handling safely while assisting with transfers using equipment like Hoyer lifts or patient transfer methods. Strong communication skills for effective client interaction and collaboration with healthcare teams. Familiarity with HIPAA regulations to ensure confidentiality of patient information. Valid certifications in first aid and CPR; additional certifications related to hospice care or memory care are a plus.
