Duties Provide compassionate, respectful, and person-centered care to clients in their homes Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and personal hygiene Assist with mobility, ambulation, positioning, and transfers using safe techniques and appropriate equipment Provide companionship and meaningful social interaction based on each client's interests and preferences Assist with meal preparation, serving meals, hydration, and other daily routines as outlined in the client's care plan Encourage and support clients in maintaining as much independence as safely possible Assist with light housekeeping tasks that are directly related to the client's care and living environment Assist clients with errands and other approved activities when included in the plan of care Provide medication reminders and other permitted assistance in accordance with the client's care plan, agency policies, and applicable regulations Support clients with dementia, Alzheimer's disease, disabilities, chronic conditions, mobility limitations, and other care needs within the scope of the position Observe and report changes in the client's physical, emotional, or behavioral condition to the appropriate Avodah supervisor Follow each client's individualized plan of care and communicate important client needs to the Avodah care team Maintain accurate and timely documentation of services provided Maintain client privacy and confidentiality in accordance with HIPAA and agency policies Maintain a safe, clean, and comfortable environment for clients Follow agency policies and procedures regarding safety, infection control, emergencies, and client care Communicate respectfully and professionally with clients, families, supervisors, and members of the care team Promote client dignity, independence, choice, and quality of life during every interaction Skills Compassionate, patient, and respectful approach to caregiving Strong communication and interpersonal skills Ability to build trusting and professional relationships with clients and their families Dependable, punctual, and responsible work ethic Ability to work independently in a client's home while following an individualized plan of care Ability to respect client privacy, personal preferences, and household routines Ability to recognize and appropriately report changes in a client's condition Understanding of confidentiality and HIPAA requirements Knowledge of basic infection-control and hand-hygiene practices Ability to safely assist clients with mobility, transfers, and positioning Comfortable providing personal care while maintaining client dignity and privacy Ability to document services accurately and communicate effectively with the Avodah care team Reliable transportation to travel to clients' homes Previous experience in home care, personal care, senior care, companion care, or caregiving is preferred CPR/First Aid certification is preferred Ability to meet the physical requirements of the position, including standing, walking, bending, and assisting clients with mobility as applicable What We Value at Avodah At Avodah Health Services, the way we care for people matters .
