Duties Prompt residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity. Monitor patient health by observing vital signs such as blood pressure, pulse, and respiration; report any concerns promptly to healthcare professionals. Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to promote health and well-being. Maintain accurate documentation of care activities, incident reports, and progress notes aligned with facility policies and resident rights. Provide companionship and emotional support, fostering a positive environment that encourages social interaction and community engagement. Qualifications Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration accurately. Knowledge of dementia care, Alzheimer's care, memory care strategies, and neurodevelopmental disorders such as autism is advantageous. Strong communication skills with the ability to engage effectively with residents, families, and team members while respecting resident's rights. Experience supporting individuals with developmental disabilities or intellectual disabilities enhances your candidacy. Ability to lift heavy objects safely and perform physical tasks such as assisting with transfers or mobility support consistently.
