Job Summary We are seeking a compassionate and energetic Direct Support Professional to join our team! In this vital role, you will empower individuals with disabilities by assisting with daily activities, promoting independence, and ensuring their safety and well-being. Responsibilities Provide direct care and support to individuals with developmental disabilities, including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility. Monitor residents' health by observing vital signs, administering medication as prescribed, and reporting any changes or concerns promptly. Assist with meal preparation, cooking, and maintaining a clean and safe living environment in group home or assisted living settings. Support residents during outings within the community, ensuring safety and engagement in social activities. Use specialized equipment such as Hoyer lifts for safe resident transfers and perform patient monitoring to ensure comfort and safety. Follow care plans meticulously while maintaining HIPAA compliance to protect residents' privacy. Document care activities accurately in resident records, including incident reports, medication administration logs, and progress notes. Skills Strong understanding of patient observation techniques, vital signs monitoring, and medication administration. Experience working with individuals with developmental disabilities, autism spectrum disorder, dementia or Alzheimer's care, memory care, or hospice care. Knowledge of HIPAA regulations and confidentiality standards related to patient information. Ability to perform heavy lifting safely using equipment like Hoyer lifts; physically capable of supporting residents' mobility needs. Excellent communication skills for collaborating with team members, residents, families, and healthcare professionals. Prior experience in group homes, assisted living facilities, nursing homes or home health environments preferred. Certifications such as CPR/First Aid are highly desirable; ongoing training in patient care practices encouraged.
