Hospice Home Health Aide (CHHA)

As a Hospice Home Health Aide, you'll provide essential hands‑on care that truly changes lives — all while being part of a supportive, mission‑driven team that values you as much as the patients we serve. What You'll Do Provide personal care and ADL support, including bathing, dressing, grooming, and mobility assistance Use Hoyer lifts and mobility devices to ensure safe transfers and movement Observe and report changes in condition, especially for patients with dementia or Alzheimer's Deliver compassionate hospice‑focused comfort care and symptom support Assist with catheter care and other tasks as trained Document all care accurately using Hospice Tools Help maintain a safe, clean home environment through light housekeeping, laundry, and meal prep Work closely with nurses and interdisciplinary team members to ensure seamless care Provide individualized support to patients with disabilities or long‑term care needs What You Bring Experience with dementia care, developmental disabilities, or memory care Strong understanding of hospice‑related medical terminology Ability to recognize and report changes in patient condition Comfort using EHR/EMR systems CPR certification (not required but valued) Experience with Hoyer lifts or other assistive devices Excellent communication skills and a compassionate, patient‑centered approach Ability to work independently while following safety and HIPAA guidelines Prior hospice or home care experience preferred Why Caregivers Love Working With Us We believe caregivers deserve the same compassion they give to others.
