Duties Assist clients with daily living activities such as personal hygiene, grooming, dressing, and meal preparation to promote independence and dignity. Support residents with mobility and transfer methods using equipment like Hoyer lifts and manual handling techniques to ensure safety for both clients and staff. Monitor patients' vital signs, observe changes in health status, and document findings accurately while adhering to HIPAA regulations to maintain confidentiality. Implement care plans tailored to each individual's needs, including medication administration, Alzheimer's care, dementia care, and memory care strategies. Facilitate participation in community activities and provide transportation assistance to appointments or outings to foster social engagement. Maintain a clean and safe environment by performing light cleaning duties and ensuring adherence to health standards. Communicate effectively with clients, families, and healthcare teams to ensure comprehensive support and uphold residents' rights. Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability in patient observation, monitoring vital signs, and recognizing changes in health or behavior. Knowledge of patient care techniques including ADLs (Activities of Daily Living), medication management, and manual handling procedures. Familiarity with specialized care such as Alzheimer's care, dementia support, autism support, intellectual disabilities support, and neurodevelopmental disorders. Proficiency in emergency response skills such as CPR and first aid; ability to perform patient transfer methods safely using Hoyer lifts or other equipment. Strong communication skills for effective client interaction and team collaboration; understanding of resident's rights is essential. Ability to perform heavy lifting when necessary while maintaining safety standards; experience with patient personal care assistance is a plus.
