This energetic position offers the opportunity to make a meaningful difference in patients' lives by assisting with daily activities, ensuring their safety, and supporting their overall well-being. As a CHHA, you will work closely with clients to promote independence and comfort while adhering to home healthcare regulations and maintaining the highest standards of patient confidentiality. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor and observe patients' health status by taking vital signs, recording medical documentation, and reporting changes to supervising healthcare professionals. Administer medications and perform catheter care following prescribed medical protocols while ensuring compliance with HIPAA and home health care regulations. Provide specialized care for clients with dementia, Alzheimer's disease, developmental disabilities, or those receiving hospice or end-of-life care. Support clients with meal preparation, light housekeeping, laundry, and cleaning to promote a safe and comfortable environment. Communicate effectively with clients and their families to ensure understanding of care plans and foster trustful relationships. Skills Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments. Strong knowledge of medical terminology, patient care techniques, and home health care regulations. Ability to accurately monitor vital signs and assist with medication administration while maintaining patient confidentiality under HIPAA guidelines. Skilled in patient transfer methods including the use of Hoyer lifts and other mobility aids. Physical ability assisting with patient transfer methods, and provide long-term caregiving support.
