Responsibilities Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support Monitor patients' health by taking vital signs and observing for changes in condition, ensuring adherence to care plans Administer medication and perform patient transfer methods safely using equipment such as Hoyer lifts and patient transfer techniques Support clients during meal preparation, feeding, and nutritional needs while maintaining a clean and safe environment Document care activities accurately in compliance with HIPAA regulations and organizational policies Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation Assist with transportation for appointments or community outings, ensuring client safety at all times Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of dementia care, Alzheimer's care, memory care, and hospice care enhances your ability to support clients effectively Familiarity with patient monitoring techniques including vital signs measurement and patient observation is essential Experience working with individuals with autism or neurodevelopmental disorders helps tailor personalized support strategies Hands-on experience with manual handling, heavy lifting, and patient transfer methods ensures safety for both clients and caregivers Background in developing or following care plans aligned with individual needs supports quality service delivery Certification in CPR and first aid is preferred to respond confidently during emergencies Join us to be part of a passionate team committed to providing exceptional home & community care.
