Key Responsibilities:
Assist clients with personal care tasks, such as bathing, grooming, dressing, and toileting. Help clients with mobility, transfers, and positioning to ensure their safety and comfort. Assist with meal planning and preparation, ensuring proper nutrition and dietary needs. Provide companionship and emotional support, engaging in conversation and activities to reduce loneliness. Monitor clients' health by observing their condition and reporting any changes to the supervisor or healthcare provider. Assist with light housekeeping duties, such as cleaning, laundry, and tidying up. Support clients with medication reminders and ensure adherence to prescribed schedules. Maintain accurate records of services provided, including daily activity logs and any changes in condition. Ensure a safe and clean environment for clients and report any safety hazards. Communicate effectively with clients, families, and healthcare team members to ensure proper care coordination.
Qualifications:
High school diploma or equivalent required. Certification as a Home Health Aide (CHHA) in New Jersey. Previous experience in home health care or a related field is a plus. Basic understanding of medical terminology and healthcare practices. Ability to lift and assist clients with mobility needs. Compassionate, patient, and empathetic demeanor. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Reliable transportation and valid driver's license (if applicable).
Working Conditions:
This position involves traveling to clients' homes within the assigned area. Flexible hours, including evenings and weekends, may be required. Physical requirements include bending, lifting, and standing for extended periods.
