Responsibilities:

Provide personalized care to patients in their homes, including assistance with daily living activities, medication management, and mobility support Develop and maintain positive relationships with patients, families, and healthcare teams Monitor patient progress, identifying and reporting changes or concerns to the healthcare team Participate in care planning and collaboration with other healthcare professionals Maintain accurate and confidential records of patient care and communications Adhere to HIPAA guidelines and maintain patient confidentiality Perform other related duties as assigned

Requirements:

Current certification as a Home Health Aide (CHHA) in the State of New Jersey High school diploma or equivalent required Ability to lift 50 pounds and move about quickly to respond to patient needs Excellent communication and interpersonal skills Compassionate and patient-focused approach to care
