Job Summary The Customized Community Support (CCS) role is designed for compassionate individuals dedicated to enhancing the quality of life for those in need of support within their communities. As a CCS provider, you will work closely with clients to develop personalized plans that cater to their unique needs and goals. This position requires a commitment to fostering independence and promoting well-being through tailored support services. Responsibilities
  • Collaborate with clients to assess their individual needs and preferences.
  • Develop and implement customized support plans that align with clients' goals.
  • Provide assistance with daily living activities, including personal care, meal preparation, and household management.
  • Facilitate community engagement by connecting clients with local resources and activities.
  • Monitor client progress and adjust support plans as necessary to ensure effectiveness.
  • Maintain accurate documentation of services provided and client interactions.
  • Advocate for clients' rights and needs within the community. Skills
  • Strong interpersonal skills with the ability to build rapport with clients.
  • Excellent communication skills, both verbal and written.
  • Empathy and patience in dealing with individuals from diverse backgrounds.
  • Problem-solving skills to address challenges that may arise during service delivery.
  • Organizational skills to manage multiple client cases effectively.
  • Experience in social services or related fields is a plus but not required.
  • Knowledge of community resources and support services available for clients.
