Duties Provide personal care assistance including bathing, dressing, grooming, and mobility support to patients with varying needs. Assist with activities of daily living (ADLs) such as toileting, feeding, and transferring using patient transfer methods. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately to Nurse Supervisor. Provide medication reminders while maintaining compliance with home healthcare regulations and HIPAA confidentiality standards. Support patients with dementia or Alzheimer's care by implementing memory care techniques and ensuring a safe environment. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a hygienic living space for clients. Document medical records meticulously and communicate effectively with healthcare teams to coordinate ongoing patient care. Skills Strong patient observation skills to detect subtle changes in health status or behavior. Knowledge of medical terminology, catheter care, and basic life support (BLS) including CPR. Ability to perform patient transfer methods safely using patient equipment Excellent client communication skills to foster trust and provide emotional support to patients and their families.
