NY Job Summary:

At Axzons Homecare, we are a team of compassionate, dedicated, and trusted home health care providers managed by healthcare professionals. We are currently looking for a professional and experienced Home Health Aide to join our team. Must have HHA/PCA Certificate. 2-3 Female HHA/PCA needed in Bayport for live in position immediately.

Duties:

• Assist patients with activities of daily living including bathing, grooming, and dressing
• Provide companionship and emotional support to patients
• Remind for medication under the direction of healthcare professionals
• Perform light housekeeping tasks and meal preparation
• Document patient care services accurately and timely
• Follow HIPAA guidelines and maintain patient confidentiality
• Assist with medical documentation and record keeping
• Provide care for individuals with developmental disabilities, dementia, in homes or in long-term care settings

Experience:

• Prior experience in caregiving or home health aide roles required
• Familiarity with working in nursing homes or similar healthcare settings
