Full Job Description Responsibilities:

• Provide direct care and support to individuals with severe and persistent mental health issues
• Assist with daily living activities, including meal preparation, personal hygiene, and medication administration
• Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors
• Foster a supportive and inclusive environment for individuals to enhance their social skills and independence
• Collaborate with a multidisciplinary team to develop and implement individualized care plans
• Maintain accurate documentation of client progress and activities
• Ensure compliance with HIPAA regulations and maintain confidentiality of client information

Qualifications:

• High school diploma or equivalent required; additional education or certification in social work, nursing, or a related field is a plus
• Previous experience in direct care or patient care preferred
• Knowledge of behavior management techniques and strategies
• Ability to lift and assist with transferring individuals as needed
• Strong communication skills and ability to work effectively within a team
• Understanding of person-centered care principles and commitment to promoting individual autonomy and choice
• Ability to use computer systems
