Responsibilities Assist residents with activities of daily living (ADLs), including personal hygiene, grooming, dressing, and mobility support using proper manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly. Support medication administration following prescribed care plans while adhering to HIPAA regulations and safety protocols. Prepare nutritious meals and assist with meal planning and preparation to meet individual dietary needs. Maintain a clean and safe environment by performing routine cleaning tasks and ensuring infection control standards are met. Document care activities accurately in resident records, including progress notes, incident reports, and updates to care plans. Provide companionship and emotional support, fostering positive communication and respecting residents' rights and preferences. Assist with transportation for appointments or community outings using appropriate safety procedures. Support residents with cognitive impairments such as dementia or Alzheimer's disease through specialized memory care techniques. Participate in training related to hospice care, neurodevelopmental disorders like autism, or developmental disabilities to enhance quality of support provided. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient observation techniques, vital signs monitoring, and patient personal care assistance. Familiarity with care for individuals with dementia, Alzheimer's disease, autism spectrum disorder (ASD), or other developmental disabilities. Ability to perform heavy lifting safely and use assistive devices such as Hoyer lifts or manual handling equipment. Excellent communication skills for effective client interaction and team collaboration. Knowledge of HIPAA regulations to ensure confidentiality of patient information at all times. Competence in emergency response procedures including CPR and first aid certification. Experience supporting patients through hospice or palliative care settings is a plus. Ability to follow detailed care plans while adapting to individual needs in a respectful manner. Skills in meal preparation tailored to specific dietary restrictions or health conditions. Capable of assisting with medication administration accurately within scope of practice. Experience working with seniors or individuals requiring long-term care support is highly valued.
